I, as the owner (or authorized agent of the owner) of the animal described above, affirm that I have the authority to provide this consent. I understand that additional medical or surgical treatment may be required based on unforeseen circumstances, and no guarantees regarding outcomes have been made. I grant authorization for the use of anaesthetics, medications, and any necessary additional treatment as determined by the veterinarian.
